State Tax question?

I e-filed my federal and ohio state tax already. But I worked in illinois for 3-4 months and had income there. I never established residency and filing it under non-resident. My problem is that I can't e-file illinois state return because it's my first time filing and you must mail it in. I think from what i gathered I need a IT1040 and a scheduling R form or something. I was told that over the phone, but it was hard to understand her. So I may be ocmpletely wrong. Does anyone know what forms I need and where I can print them.

1. did you check the Illinios tax agency's website? if the forms aren't downloadable, they'll at least have the telephone number to get them.
