does anyone know what irs forms I need?

I have already filed my 2005 income tax and I have recieved my refund, but I am from New Orleans and I just found out thru a friend that I can file for my Katrina. Does anyone have a clue as to how I file for those losses? Do I need to refile with my income like normal or can I just file for my losses?

1. File a 1040-X Individual Amended Tax Return. Download it from irs.gov and follow the instructions.
2. You would need to file a 1040X, an amended tax return. Column A is the return you originally filed, Column C is what a correct return would have looked like, and Column B is the reconciliation between the two.
